Clinical Advisor Story

Trust begins here

Sarah is a Clinical Advisor for LMW and Registered Nurse, providing Member advice and support to help ensure safe, high-quality care for our Members. Sarah supports complex care planning, health management and clinical assessments.

“I’ve always been passionate about healthcare. I worked as a nurse in both mental health and intensive care but what really drew me to disability work was the opportunity to build deep, ongoing connections.”

“I love visiting Members in their homes, and I get excited about doing their health assessments so I can really understand their support needs.”

“There can be a real disconnect between disability and healthcare and sometimes we discover things that weren’t in any reports.” This means Sarah’s nursing background helps her spot clinical gaps others might miss — giving Members more personalised care.

“One of the Members I supported was going through a very tough time: her health had started to decline quickly, and she was in a lot of discomfort. She didn’t want anyone taking away her choice or independence.”

“It was like watching someone move through the stages of grief: she was coming to terms with the fact that she couldn’t manage everything on her own anymore.” This is a common reaction for all of us... fear and resistance can impede acceptance and trust.

“At first, she had a number of providers involved, which was difficult to navigate. We had to figure out how to work together while still respecting her choices.”

“A beautiful moment came when I was able to step in and support her with a clinical procedure. She was in pain and didn’t have the right support in place. The hospital hadn’t arranged any follow-up, so I went to her home and stayed with her, working with the paramedics to make sure everything could be handled there, just as she wanted. She was very clear: she did not want to go back into hospital.”

“At the end of it all, she said thank you. This was huge. It told me that, in that moment, I’d earnt her trust as we had supported her to remain in the comfort of her own home, just as she wanted.”

“This is one of the real sweet spots in my work — I knew she’d let her barriers down.” Trust can take time to build, but it changes everything –support becomes more personal, more effective, and more in tune with what really matters to the person.
