Providing Your Personal Information to Living My Way
In order to provide services and supports and to conduct our business activities, we will need to collect some of your personal information. This includes your name, home address, email address and phone number. We may also collect some sensitive information such as information about your health or finances so that we can plan and organise you with the best services that meet your needs. We will respect and understand if there are highly sensitive information that you don’t wish to share.
We take reasonable steps to ensure the personal information we collect about you is accurate, complete, up-to-date, relevant, secure and that it is not accessed or disclosed to anyone without your consent. We will only disclose your information to relevant people within our organisation who may need to know that information to provide you with the best services. We can only do this in ways that comply with the Privacy Act.
Correcting Your Personal Information
Under the Privacy Act, you have the right to ask for access to personal information that we hold about you and ask that we correct that personal information.
You can ask for access or correction by contacting us and we must respond within 30 days. If you ask, we must give you access to your personal information, and take reasonable steps to correct it if we consider it is incorrect.
